What a Normal ER X-Ray Can—and Cannot—Tell You After a Charleston Car Accident
You went to the emergency room after a car wreck in Charleston. Your scan showed no broken bones, and you were able to go home. That was welcome news.
But now, turning your head hurts. Sleep is harder. Getting through a workday takes more effort.
How can you still hurt when the scan looked normal?
An X-ray or CT scan can help check for serious bone injuries. It does not rule out every injury that can cause neck pain. Your symptoms, exam, and scan results guide the next step.
What your ER scan can tell you
The ER team checks for injuries that need urgent care. Imaging is one part of that evaluation.
X-rays show bones and their alignment. They can help reveal a fracture or a bone that has moved out of place. CT scans give a more detailed view of bone. For adults who need neck imaging after a significant injury, CT is often the recommended first test. Some people do not need imaging at all.
Check your discharge papers to see which test you had. “No fracture” is a useful finding, but it does not explain every possible source of pain.
Why pain can remain when no bones are broken
Your neck also has muscles, ligaments, discs, and small joints. These tissues help support your head and allow it to move.
A sudden force during a car wreck can injure some of those tissues:
- A sprain is an injury to a ligament, which connects bones.
- A strain is an injury to a muscle or tendon.
- Discs and nearby nerves may also need assessment, depending on your symptoms and exam.
A plain X-ray does not directly show a muscle strain or ligament sprain. CT has limits when assessing these tissues, too.
That does not mean everyone with pain has a hidden, serious injury. It means we need to connect the scan with what is happening in your body.
When an MRI may help
MRI gives a closer look at discs, ligaments, the spinal cord, and nerves. It may help when the exam raises concern about an injury involving those structures.
Pain alone does not automatically mean you need an MRI. A clinician considers your symptoms, strength, feeling, and other findings before choosing the next test.
An MRI also needs to answer a useful question. Will the result change your care? Do the findings fit your symptoms? More images do not replace a careful exam.
What I look for at a follow-up visit
At Elite Family Chiropractic in West Ashley, I begin with what happened during the wreck and what has changed since then.
I review available ER records and imaging, then check movement and relevant nerve function. Depending on your symptoms, that may include strength, reflexes, and feeling in your arms or legs.
I also ask about daily tasks. Can you turn your head to drive? Sit through work? Sleep without waking in pain? These details help us understand the problem and track your progress.
My fellowship training in spinal biomechanics and trauma helps guide how I bring your history, exam, and imaging together. Before we begin treatment, I explain what the findings suggest and whether you need more testing or another provider.
How the findings guide your care
Care depends on the injury and what is safe for you. A plan may include appropriate chiropractic care, exercises, and changes to daily activity.
Some patients need help from physical therapy, pain management, or another medical provider. Others need more testing before hands-on care is appropriate.
We follow your progress and change the plan when needed. Our goal is to help you understand the problem and move toward the care you need.
Warning signs that need urgent care
Return to the ER or call 911 for new weakness, trouble walking, new trouble passing urine, loss of bladder or bowel control, a worsening severe headache, repeated vomiting, or confusion after a wreck. An earlier normal scan does not cancel out new warning signs.
